  1. Print off the application form
  2. Complete, sign and date page 1
  3. Complete page 2 (Annexe to Part 1) up to and including question 9
  4. Fax to Kidshak Agency (020 8303 2730), or email to clients@kidshak.com
  5. Kidshak will then complete the remainder of this application form

Childrens Performance Regulations 1968
Children under 16 years of age require a licence for paid performances. It is the clients responsibility to licence the children and the Agency are not to be held responsiblefor the clients duty to licence.Please note to put a licence in place takes a minimum of 7-10 working days.
