  1. Print off the application form
  2. Complete, sign and date page 1
  3. Complete page 2 (Annexe to Part 1) up to and including question 9
  4. Fax to Kidshak Agency (020 8303 2730), or email to clients@kidshak.com
  5. Kidshak will then complete the remainder of this application form

Childrens Performance Regulations 1968
Children under 16 years of age require a licence for paid performances. It is the clients responsibility to licence the children and the Agency are not to be held responsible for the clients duty to licence. Please note to put a licence in place takes a minimum of 7-10 working days.
